Fan is a 23-year-old gentleman from Guangzhou, who has just successfully completed his undergraduate studies in California, USA. He has obtained a satisfactory job in CA.

Fan began to have right leg pain, and went to a local clinic since Dec. 2022. He noticed that his thighs were swollen on 20 Dec. , so he went to the Emergency Room(ER).He had dyspnea and chest pain after arriving at the hospital, and suddenly lost consciousness half an hour later. ER nurses and doctors found that Fan had cardiac arrest and called code blue immediately, and perfomed cardiopulmonary resuscitation (CPR). The heartbeat recovered after 3 minutes of CPR, but soon it stopped again, so the ER doctor urgently did trachea intubation and V-A ECMO catheterization under CPR. It was found that the culprit was pulmonary artery embolism after chest CT scan.He was transferred to the ICU for further treatment.

Fan's cardiac function was good and ECMO was withdrawn on December 22. However, Fan developed ischemic brain injury due to long-term CPR, and continued to be treated with a ventilator for endotracheal intubation.

It was peak of the COVID-19 epidemic in China in December.Fan’s parents were anxious to know about his illness, but they could not go to United States due to epidemic control.

China will manage COVID-19 with measures against Class B infectious disease,instead of Class A infectious disease,in a major shift of its epidemic response policies on 8 Jan. 2023.COVID-19 testing and centralized isolation for all overseas travelers entering China will be canceled,but those with a normal health declaration and no abnormality in routine quarantines at the customs port can be greenlighted to social mobility,according to the new policy.

Fan's parents contacted the TKP team and performed request to transfer their son from Sen Diego to Guangzhou for further treatment.

The TKP team members communicated with the attending physician in United States.Fan could not regain consciousness and spontaneous breathing in a short period of time due to ischemic hypoxic encephalopathy.All of them agreed that tracheostomy could be performed and ventilator-assisted ventilation could be used. TKP member coordinated the arrangement of a medical charter flight back home on 7 Jan. 2023.
